Print this form and take a copy to your local formal wear store where you will be measured. Enter your measurements below and complete the registration process.

Tuxedo Avenue cannot be held responsible for measurements not taken by Tuxedo Avenue or for tuxedos not tried on in one of our Tuxedo Avenue locations.

A non-refundable $40 reservation down payment is required before your tuxedo can be ordered. Submit your payment information by this form or request a Tuxedo Avenue Associate call you for payment information.

Rental Agreement

I authorize Tuxedo Avenue to charge my credit card and / or Tuxedo Avenue will take legal action against me for full retail value of all items not returned or damaged beyond rental reuse condition. I also understand a penalty of $20.00 / day will be charged if items are not returned the next day following the wear date, up to a maximum of three days, after which Tuxedo Avenue reserves the right to charge full replacement cost. Replacement cost: Coat- $300, Pant- $100, Tie- $25, Shirt- $30, Vest- $100, Shoes- $80, ( Suspenders- $25 if not included in complete tux/suit replacement ). Complete Tuxedo / Suit replacement cost- $635. Complete and proper delivery of the ordered items requires a final fitting of each order at least (24) hours prior to the event. If the customer declines the final fitting ( also assumed if someone else is authorized to pick up the order ), Tuxedo Avenue cannot guarantee the fit of the ordered items and will not consider fit issues as a reason for refund. The initial minimum payment of $40 ( required to reserve your order ) is nonrefundable and applied to customer balance ( not a separate refundable deposit ).

            Tuxedo Avenue will not be held responsible for measurements not taken at a Tuxedo Avenue company store. The customer will be responsible for shipping charges.
